Front Desk Policies & Procedures
1. Appointments may be scheduled during regular office hours at either location. Please inform the receptionist or nurse of the nature of your visit so adequate time is planned for your appointment. Shot patients do not need an appointment. The staff will efficiently handle you when you arrive at a time of your convenience.
2. To cancel an appointment, please contact us 24 to 48 hours prior to your scheduled visit. We appreciate the notice so that others might utilize the appointment time.
1. New patients are asked to come early to their first appointment. There are various forms concerning past medical and family history that need to be completed. Most of these forms can be found on the website, ie (Forms page) or click here to link
directly. Click here... New Patient Forms Minors must be accompanied by a parent or legal guardian at the first visit and all subsequenct office visits. Please bring the following with you:
List of all medications and over-the-counter drugs you are currently taking
Insurance card and picture ID
Referral Letters - If required and you do not have it you can not be seen
2. Remember, if you are an established patient and have not been seen in over 3 years, you must again be scheduled as a new patient appointment.
1. Please bring your most current insurance card so our staff may verify and update any changes.. Also,
please remember to inform the staff if you have a new address, phone number or name. If your insurance requires a referral or co-payment please have them ready at time of service
